Scope of works
The Council intends to appoint a suitably qualified, CQC-registered provider to deliver care and support across six Extra Care Housing schemes within the Oxfordshire area comprising of approximately 220 homes. The service will include a 24/7 on-site core response for emergencies and unplanned needs, alongside flexible planned care commissioned following individual Care Act assessments. Requirements may also include wellbeing checks, support for new residents, step-up and hospital-discharge pathways, activities promoting independence and social inclusion, partnership working, safeguarding, workforce continuity, performance reporting, innovation and continuous service improvement.
