Scope of works
WM5G Ltd is seeking to procure a configurable Digital Discharge Service (DDS) Workflow solution to support the coordination of hospital discharge-to-home pathways across NHS partner organisations. The solution will provide a single operational platform through which WM5G Ltd staff can manage referrals, patient workflows, consent, tasks, communications and service performance, replacing fragmented processes that currently rely on multiple supplier systems and manual administration. The procurement covers the software platform, implementation, integration, hosting, training, support and maintenance required throughout the contract term. The solution will act as the operational orchestration layer for the wider DDS service, providing a unified view of patient journeys while integrating with existing Home Readiness, Remote Monitoring and Asset Management services. It is intended to improve the efficiency, scalability and governance of discharge services while maintaining clear accountability and visibility across multiple organisations and care settings. The platform must support the complete discharge workflow, from patient identification and referral through triage, enrolment, discharge preparation, discharge, monitoring and completion. Core capability areas include referral and onward-care management, workflow and task management, consent management, reporting and analytics, administration and role-based access control, standards-based integration, and data quality management. WM5G Ltd has defined a mandatory high-level architecture that suppliers must conform to. The solution must support API-first integration, HL7 FHIR interoperability standards, UK cloud hosting, strong identity management and secure integration with external systems. The architecture is designed to support rapid onboarding of new customers and providers while minimising operational complexity and vendor lock-in. The authority's desired outcomes are improved patient flow, reduced discharge delays, better patient and carer experience, reduced administrative burden, improved operational oversight, higher data quality and a platform capable of scaling across multiple regions and thousands of patients. Compliance with NHS and regulatory standards, including DCB0129, DCB0160, DTAC, DSPT, Cyber Essentials Plus and UK GDPR, is mandatory. Contract award will be based on the Most Advantageous Tender (MAT), assessing suppliers' ability to meet both the mandatory architectural requirements and the operational outcomes defined within the Requirements Compliance Matrix.
