PQQ in this case is where the City, at an early stage in the development of its proposals but prior to formulating any formal procurement opportunity, seeks input from the market. Participants must also note that a response to this notice does not guarantee an invitation to participate in this or any future procurement that the City may conduct, nor that the City will procure any such supply and services or accept any proposals offered. Any Procurement of the supply and services by the City will be carried out strictly in accordance with the Public Contracts Regulations 2015. Description of Service The Health Access Worker service will improve access to healthcare for people sleeping rough by providing wraparound connections for rough sleeping clients into the health system. The service will provide lived experience as a core component of provision. The Health Access Worker will: Improve access to healthcare for people sleeping rough by providing wraparound connections for rough sleeping clients into the health system. Be headquartered at the Outreach provider’s office, but with the understanding that it is peripatetic/out in the field for a primary portion of the workday. Provide lived experience as core component of service provision. Be expected to work 3 days per week and to meet with clients outside business hours as needed. Improve access to healthcare for people sleeping rough. Increase the number of GP registrations. Increase attendance at health appointments. Reduce missed GP and secondary healthcare appointments. Prevent deterioration of health to reduce escalation to crisis point and minimise risk of long-term impact of rough sleeping on physical health. Be part of a whole-system approach that recognises the interdependence of health, housing and social inclusion. Improve uptake of outreach primary care offer. Increase individuals’ confidence, knowledge and ability to take an active role in managing their own health and wellbeing, empowering them to engage effectively with treatment, appointments and longer-term health support. Maximise use of Health Emergency Bedspaces when available. Support the implementation and evaluation of a City of London-led NHS information-sharing pilot. Improve early identification of long-term conditions that qualify people for support through health funded palliative care. Support embedding inclusion health principles within outreach services, ensuring that health is a core component of their support. Amplify the voice of lived experience in homelessness work. Play a key role in both housing stability and health outcomes for individuals identified as long-term rough sleepers, ensuring access to accommodation and services that meet their individual health and care needs. Work collaboratively with partner agencies and stakeholders to deliver the objectives set out in the MHCLG strategy and CoL’s Long Term Rough Sleeper partnership plan. Management (including direct line management) of HAW workers will be the responsibility of the provider. The contract will run for an initial period of one year with the option to extend by one year. The maximum value of the contract will be £40,000 per annum.
