Scope of works
The befriending Service will offer supportive, reliable relationships through Volunteer befrienders to individuals age 65+ living in Calderdale who would otherwise be socially isolated. The aim of the Service is to: •Alleviate social isolation in older people age 65+ with long term conditions including people with physical disability, sensory impairment, early onset dementia and people moving out of hospital •Enhance the quality of life for the individual using the Service, their family, and carers •Improve health and social outcomes for people •Improve emotional wellbeing •Ensure the Service is well advertised to maximise usage •Take a person-centred approach with the individual building on their strengths •Recognising the individual's skills, abilities and things that are important to them in their life and community
