Scope of works
This Negotiated Request for Proposal (RFP) is an invitation from Mohawk Medbuy Corporation (hereinafter referred to as “MMC) to Proponents to submit proposals for the provision of Hemodialysis Access products; for a contract term of three (3) years with an option to extend for one (1) additional period of two (2) years. Proponents must be able to meet or exceed all the requirements of this RFP for a Hospital-by-Hospital award, for the following categories: Chronic Dialysis Catheter Kit and Related Accessories Fistula and Graft Needles Cannulas and Related Accessories Acute Dialysis Catheter Kit and Related Accessories Dialysis Connectors and Related Accessories Proponents must have the breadth of product to meet the commitment level in order to be in consideration for the committed portion of the award for the category.
